The distribution of fat tissue is established at puberty, following genetic and hormonal patterns. Localised fat is not a failure of effort: it is biology. Liposuction is the answer to localised fat in skin with reasonable tone. Abdominoplasty is the answer when the main change is excess skin and separated rectus muscles. Brachioplasty, thigh lifts and buttock lifts are the answers when skin laxity predominates, typically after major weight loss. Working out which problem you really have is the whole job of the consultation.

A patient who asks for liposuction, but whose main anatomical change is skin laxity, will be disappointed by liposuction. A patient who asks for an abdominoplasty, but whose main change is localised fat without excess skin, may be well served by liposuction alone. A patient with lipoedema needs a procedure designed for lipoedema, not standard cosmetic liposuction.

None of these procedures is interchangeable, and receiving the wrong operation is the most common cause of dissatisfaction in body contouring as a discipline. Technique is chosen for anatomy and indication, not for trends.

Is liposuction a weight-loss procedure?

No. Liposuction removes localised, exercise-resistant fat deposits in patients who are close to their stable weight. In patients who are far from a stable weight, the body redistributes fat after surgery and the result will not last. We say this clearly during the consultation and, in some cases, recommend a period of supervised weight management before any surgical step.

In my case, is liposuction enough or do I need an abdominoplasty?

What is lipoedema, and is it the same as obesity?

Lipoedema is a chronic disease of the subcutaneous fat tissue, almost exclusively affecting women, which causes symmetrical, disproportionate and often painful fat accumulation in the legs (and sometimes the arms) that does not respond to diet and exercise. It is not obesity (although the two conditions can coexist) and it is frequently misdiagnosed for years. Diagnosis is clinical and by stage. Treatment combines conservative therapy (compression, manual lymphatic drainage, exercise) with a staged surgical pathway that uses specifically adapted liposuction techniques.

Does liposuction leave scars?

Liposuction is performed with fine cannulas and minimal access points, and does not involve the long scar of procedures that remove skin. Body surgery procedures other than liposuction alone, however, do leave a scar: if your plan combines liposuction with an abdominoplasty, a brachioplasty or a thigh lift, the position of that scar becomes a planning priority and is discussed at the consultation. Scars mature over twelve months, and scar quality is partly down to biology.

How long is the recovery after liposuction?

Allow about a week off work. As with other body surgery procedures, the pathway involves a week to ten days of reduced social activity, with the compression garment worn as instructed, check-ups scheduled at the intervals your procedure requires and a direct line to your surgeon in the meantime. Liposuction of the abdomen and flanks is performed as day surgery; the small areas (neck, double chin and arms) can also be treated under local anaesthesia with sedation. In the other cases, too, day surgery is the planned setting. The timescales that apply to you depend on the areas treated and the expected volume, and are set out in the recovery calendar you take home with your plan.

Will the results of liposuction last?

For a procedure performed on a patient close to their stable weight and in a stable life situation, yes: the change in contour is lasting. Fat distribution is decided by genetics, and the deposit treated is precisely the one that diet and exercise could not shift. If, on the other hand, you are still actively losing or gaining weight, the body redistributes fat after surgery and the result will not hold; the same applies if a pregnancy is planned in the near future. We discuss this at the consultation, and in some cases it is the reason why it is better to postpone the surgical step.

Can I have several areas treated in the same session?

Often yes, when the total operating time, the overall volume of liposuction and your health allow it: flanks and abdomen, thighs and knees, arms, neck and double chin can be included in a single procedure, and the aspirated fat can be reinjected with fat grafting (lipofilling) where volume is needed. Each additional area, however, lengthens the operating time and increases the physiological load, and beyond a certain threshold the combined risk profile outweighs the benefit. How many areas to treat together, and whether to add fat grafting, is decided case by case during the consultation, not in advance.

How much does liposuction cost?

It depends mainly on how many areas are treated and which ones, and therefore on the time in theatre: prices start from €3,500 for a procedure of about half an hour and go up to €14,000 for procedures of more than three hours across several areas, all inclusive; the same applies if fat grafting is combined with the reshaping. The small areas can also be treated under local anaesthesia with sedation. Once the procedure has been recommended, you receive a detailed written quote, with no hidden costs.
