Why some changes need surgery — and which ones don’t
Adipose tissue distribution is set in puberty by genetic and hormonal patterns. Localised adiposity is not a failure of effort; it is biology. Liposuction is the answer to localised fat in reasonably toned skin. Abdominoplasty is the answer when the principal change is excess skin and a separated rectus muscle. Brachioplasty, thigh lift and gluteal lift are the answers when skin laxity is the dominant feature — typically after major weight loss. Distinguishing which problem you actually have is the entire job of the consultation.

Clinical examination, measurements, photographic documentation, an anatomical analysis — skin laxity, fat distribution, muscle wall — and your goals heard in your own words. You leave with a written plan and an itemised cost, not a contract.

Time to decide
Major operations are rarely scheduled at the first visit. You take the plan home, read it, sleep on it, ask questions, come back if you need to. There is no countdown.

A patient asking for liposuction whose principal anatomical change is skin laxity will be disappointed by liposuction. A patient asking for a tummy tuck whose principal change is localised fat without skin excess can be served by liposuction alone. A patient with lipedema needs a procedure designed for lipedema — not standard cosmetic liposuction.
None of these is interchangeable, and getting the wrong operation is the most common cause of dissatisfaction in body contouring as a discipline. Technique is chosen by anatomy and indication, not by trend.

Frequently asked questions
about body surgery
Is liposuction a weight-loss procedure?
No. Liposuction removes localised, exercise-resistant fat deposits in patients who are near their stable weight. In patients far from a stable weight, the body redistributes fat after surgery and the result will not last. We say so plainly during consultation, and in some cases we recommend a period of supervised weight management before any surgical step.
What is the difference between liposuction and a tummy tuck (abdominoplasty)?
Liposuction removes fat from a region whose skin still has good elasticity. Abdominoplasty removes redundant skin and tightens the abdominal wall, and includes a substantial scar designed to be hidden under underwear. Many post-pregnancy and post–weight-loss patients need a tummy tuck rather than liposuction; many patients with localised fat and good skin need liposuction rather than a tummy tuck. The right operation depends on what your principal anatomical change actually is.
What is rectus diastasis, and how do I know if I have it?
Rectus diastasis is a separation of the two rectus abdominis muscles down the midline, frequent after pregnancy. It produces a visible central bulge or doming when you sit up from lying down or when you bear down. A simple physical test during consultation, and ultrasound where useful, confirms the diagnosis and width. Mild diastasis often improves with appropriate post-natal physiotherapy in the first year. Persistent, wide, or symptomatic diastasis is a surgical indication.
What is lipedema, and is it the same as obesity?
Lipedema is a chronic disease of the subcutaneous adipose tissue, almost exclusively in women, that produces symmetrical, disproportionate, often painful adiposity of the legs (and sometimes the arms), unresponsive to diet and exercise. It is not obesity — although the two can coexist — and it is frequently misdiagnosed for years. Diagnosis is clinical and stage-based. Treatment combines conservative therapy (compression, manual lymphatic drainage, exercise) with surgical staging using specifically adapted liposuction techniques.
Will I have visible scars?
Yes. Body contouring procedures other than pure liposuction leave scars. Scar placement is a planning priority — low transverse and hidden under underwear for abdominoplasty, in the inguinal fold for thigh lift, on the inner upper arm for brachioplasty, in the inframammary or peri-areolar region for gynaecomastia. Scars mature over twelve months. Scar quality is partly biological. The thigh-lift scar in particular is, as the department’s published columns note, not always of optimal quality.
How long is the recovery?
Liposuction: about a week off work. Mini-abdominoplasty: ten days to two weeks. Full abdominoplasty: two to three weeks before light work; six weeks before impact activity; full scar maturation in twelve months. Brachioplasty and thigh lift: two weeks to light activity. Lipedema surgery is staged; each stage has its own timeline. Each plan you take home includes a written, procedure-specific recovery schedule.
Will the result last?
For procedures performed in patients near their stable weight and stable life situation, yes — the contour change is durable. For patients still actively gaining or losing weight, or for women planning a future pregnancy in the near term, the result will be modified by those subsequent changes. We discuss this at consultation.
What about gynaecomastia in adult men?
Gynaecomastia (enlargement of the male breast) has multiple causes — hormonal, pharmacological, idiopathic — and a medical work-up is performed before surgery to exclude reversible causes. When the gynaecomastia is established and the pre-operative work-up is clear, surgical correction is one of the most reliably satisfying procedures in body contouring. Most cases combine peri-areolar glandular excision with surrounding liposuction.
Can I combine procedures in one operation?
Sometimes, when total operative time, total volume of liposuction, and patient health allow it. Combining procedures is convenient — one anaesthetic, one recovery — but each combined procedure adds operative time and physiological load, and beyond a certain threshold the combined risk profile exceeds the benefit. The right combination is decided case by case during consultation, not in advance.
What does it cost?
Cost depends on the procedure, operating-room time, anaesthesia, and case complexity. After your consultation, once a procedure has been recommended, you receive a written, itemised estimate with no hidden fees. The department does not negotiate on price and does not run surgical promotions.
