Did you know?

The idea of using fat for the breast is more than a century old. In 1895 the surgeon Vincenz Czerny removed a lipoma from the lower back and transplanted it to the chest to reconstruct a breast. The implant with a shell filled with silicone gel arrived only in the early 1960s, introduced by two Houston surgeons, Frank Gerow and Thomas Cronin.

If expectations or the requested implant size do not meet the criteria of proportion and anatomical safety, the surgical indication is put on hold. This represents a pause for assessment while waiting for suitable clinical conditions to be in place.

The technical choices in breast augmentation

Augmentation can be performed with silicone gel implants (anatomical or round) or, in selected cases, with fat grafting (lipofilling) using your own fat. When there is sagging, augmentation should be combined with a breast lift (mastopexy): an implant alone does not correct ptosis.

Choosing the technique is the decisive step: implant plane, incision site and implant type are defined on the basis of your anatomical parameters, not from a catalogue.

At Palazzo Manzoni, breast augmentation with implants starts from €10,000, implants included. If the breast also needs lifting (augmentation with mastopexy), the price ranges from €11,000 to €13,000. Prices include the surgeon, anaesthesia, the operating theatre, an overnight stay where needed, the implants and follow-up visits after surgery.

The final quote is drawn up at the consultation, in writing, once the surgeon has assessed your case and agreed the most suitable technique with you. How is breast augmentation performed?

How is the implant chosen?

The choice of implant is based on rigorous anthropometric parameters: chest width, breast base diameter, shape of the inframammary fold, thickness of the skin and fat envelope and quality of the soft tissues. During the consultation, the patient assesses suitable volumes using dedicated trial bras. The guiding criterion is never the maximum volume the body can tolerate, but long-term structural sustainability and aesthetic harmony.

Do implants last forever?

No. Latest-generation implants last for decades, but over time a revision or replacement may be needed (due to wear, anatomical changes, capsular contracture or personal choice). We always discuss this openly before any operation.

Will I be able to breastfeed after breast augmentation?

After breast augmentation through an inframammary fold incision, breastfeeding is usually preserved. With a periareolar or armpit incision, and when augmentation is combined with a breast lift, the answer depends on the technique and your anatomy, and cannot be guaranteed. If breastfeeding in the future is a priority for you, tell us at the first consultation: this need will guide the choice of incision site and implant plane, which nonetheless remain an outcome of the consultation and never a starting point.

Will I lose nipple sensation after breast augmentation?

A temporary change in sensation is common in the first weeks after augmentation. Persistent loss is rare with an appropriate technique, but remains possible, particularly with the periareolar approach. Since the incision site and implant plane are defined during the consultation on the basis of your anatomical parameters, the risk profile of the technique chosen for you is quantified and explained as part of pre-operative consent.

What is capsular contracture, and how common is it?

The formation of a thin fibrous capsule around the implant is the body’s natural response and part of the normal healing process. In a small percentage of patients, however, this capsule can contract, making the breast feel firmer or changing its shape. Although latest-generation implants and modern surgical techniques have drastically reduced this phenomenon compared with the past, the risk cannot be eliminated entirely, and it remains the main cause of revision in breast augmentation. Management ranges from simple monitoring in mild cases to capsulectomy with implant replacement in more advanced cases.

And what about BIA-ALCL or “breast implant illness”?

BIA-ALCL is a rare form of lymphoma associated with specific models of textured implants; the types at greatest risk have been withdrawn from the market in many countries, and the incidence with the implants currently in use is extremely low. “Breast implant illness” (BII), on the other hand, is the term used for a set of systemic symptoms reported by some patients; the scientific evidence on the subject is constantly evolving and a direct link with implants has not yet been fully established. During the consultation we discuss these topics with complete transparency, sharing the most up-to-date scientific information.

How long is recovery after breast augmentation?

The operation takes place in our Day Surgery theatres and most patients go home the same day or after one night of observation. A return to desk-based work is generally expected within 7-10 days; for high-impact sports, clearance usually comes at around 6 weeks, subject to clinical assessment. When augmentation is combined with a breast lift, timings are specified during the consultation on the basis of the actual plan. Check-ups are scheduled at the intervals your operation requires, with a direct line to your surgeon and their team if anything worries you in the meantime.

Will breast augmentation scars be visible?

Yes, a scar remains, which is why its placement is one of the cornerstones of planning: in the inframammary fold, around the areola or in the armpit, depending on the incision site chosen for you during the consultation. When augmentation is combined with a breast lift, the incision pattern is more extensive, because it also includes repositioning. Scars complete their maturation over twelve months, becoming progressively paler and softer than in the early stages. Since the final appearance of a scar also depends partly on individual biological factors, we will give you detailed instructions for care at home and monitor how it develops at every follow-up visit.

Can fat grafting replace an implant?

Fat grafting allows more modest increases in volume (typically a fraction of a cup size per session) and is suitable for patients with enough donor fat who accept a gradual augmentation and the natural partial reabsorption of the grafted fat. It is a valid alternative for those who wish to avoid implants, as well as a valuable complement to implant procedures and breast reconstruction. However, it should not be considered a substitute for breast augmentation with implants for those who want a more significant increase in volume.
